Key Responsibilities
- Deliver personalised wellbeing support tailored to each individual’s needs and circumstances.
- Manage a varied caseload, ensuring high-quality support and achievement of agreed outcomes.
- Complete comprehensive assessments, including screening for Learning Difficulties and Disabilities (LDD).
- Develop strengths-based action plans that support sustainable progress and positive lifestyle change.
- Build strong relationships with local statutory, voluntary and community organisations, making appropriate referrals and participating in multi-agency case conferences.
- Deliver support through a range of methods, including one-to-one appointments, group workshops, face-to-face sessions and digital platforms.
- Plan, facilitate and evaluate engaging group sessions that meet the needs of service users.
- Maintain accurate records and ensure all work is delivered in line with contractual requirements, quality standards and safeguarding responsibilities.
- Actively engage in continuous professional development, using feedback and reflective practice to continually improve service delivery.
About You
We’re looking for someone who is compassionate, resilient and committed to achieving positive outcomes for people with complex needs. You will have:
- Recent experience supporting individuals to improve their personal wellbeing, ideally within a criminal justice or related setting.
- Experience managing a caseload and delivering person-centred support to achieve meaningful outcomes.
- The ability to develop effective strengths-based action plans that motivate and empower individuals.
- Knowledge of local health, wellbeing and support services across statutory and community sectors.
- An understanding of quality standards and best practice within justice or community rehabilitation services.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with individuals, colleagues and partner organisations.
- A commitment to continuous learning, reflective practice and professional development.
Skills and Experience
The successful candidate will demonstrate:
- Experience working in a target-driven environment while maintaining a person-centred approach.
- Confidence delivering both one-to-one support and group sessions.
- Experience supporting individuals within a criminal justice or related environment.
- Strong digital skills and experience delivering services through online platforms.
- Counselling or coaching skills relevant to supporting health and wellbeing.
- Mental Health First Aid and Suicide Awareness/Prevention training (or willingness to work towards these).
- Experience supporting individuals affected by domestic abuse, including victims and/or perpetrators.
- Substance Misuse Awareness Level 3 qualification (desirable).
